In addition, BCA-StoneCrest Center works very closely with referral source assigned “Hospital Liaisons” that follow their clients from admission to discharge, attend treatment team meeting, and provide additional historical patient information and insights. These relationships are very helpful during the patient’s hospital stay and the coordination of wrap-around services after discharge.
